The longevity Rx category is fragmented. A handful of telehealth services prescribe rapamycin off-label, NAD+ injections, sermorelin, metformin, low-dose naltrexone. Some are legitimate; some are wellness marketing with thin medical oversight. Here's how to vet them.

The dad-test answer in three lines

  1. Most longevity Rx prescribing is off-label. Rapamycin is FDA-approved for organ transplant patients; longevity dosing is off-label. Metformin is FDA-approved for type 2 diabetes; geroprotective use is off-label. Sermorelin is FDA-approved for pediatric growth hormone deficiency; adult longevity use is off-label. None of this is illegal - off-label prescribing is standard medical practice - but it means the prescribing physician's judgment is doing more work than the FDA label. Pick a service with real medical oversight.
  2. How we score longevity Rx services

Seven axes that reflect actual longevity-prescribing risk, not marketing copy.

  • Prescriber model. Real US-licensed physicians reviewing and prescribing - not a "doctor on demand" placeholder. Bonus: physicians with longevity-medicine training or board certification beyond primary care.
  • Lab integration. Baseline biomarkers and follow-up labs required before / during treatment (especially for rapamycin, metformin, LDN). Services that skip labs for "convenience" are skipping the medicine.
  • SKU breadth. Rapamycin only? Or rapamycin + metformin + NAD+ + sermorelin + LDN + the rest? Broader menu means one provider covers more of your stack.
  • Compounding pharmacy partnership. Named 503A or 503B pharmacy. The pharmacy backstops the supply chain. Unnamed compounders are a red flag.
  • Off-label prescribing transparency. The service tells you upfront when a drug is off-label and why. Vague "longevity protocols" without naming the regulatory status is marketing.
  • Pricing transparency. Prices visible without account creation. Subscription terms clear (monthly, quarterly, auto-renew).
  • Operator track record. Years in operation, public-facing leadership, LegitScript or equivalent. Avoid: brands launched in 2025 with no founders named.

Pick this if: You want injectable NAD+ as part of a longevity stack. Most retail NAD+ supplementation is oral NMN / NR precursors with debated bioavailability; injectable NAD+ delivers the molecule directly. Ageless prescribes it alongside their broader longevity menu (TRT, sermorelin, GLP-1).

Watch: NAD+ injection (vs oral NMN/NR) has limited Phase 3 human RCT data despite being widely used. Real research exists, but the longevity claim is mechanism-plus-anecdote, not RCT-confirmed. Verify the clinician is comfortable with the off-label longevity framing at intake.

Best for: Readers building a longevity stack who want injectable NAD+ under prescriber supervision rather than self-administered. Pair with TRT or sermorelin for compounding effect across the GH-axis / mitochondrial axes.

Best for: The reader who wants a single transparent monthly price for a longevity Rx and doesn't need extensive biomarker tracking.

  • Doctors Studio. Concierge longevity medicine, higher price point, more direct physician access. Not typically affiliate-program operated.
  • Rx HealthMed. Rapamycin-focused with broader peptide menu. Pricing per cycle. Worth checking if rapamycin-specifically is your interest.
  • Maximus Tribe. Originally TRT-focused (see our TRT matrix), expanded into broader hormone and longevity menu. Founder is public-facing.
  • Truemed-enabled brands. Levels Health, AG1, etc. - not direct longevity Rx providers but adjacent. HSA-payable through Truemed at checkout where eligible.

The Skip Tier - what we will not link, and why

  • Wellness clinics selling "longevity stacks" alongside Botox + fillers. Aesthetics-led practice is not the same as longevity medicine. The cross-sell pressure to the high-margin cosmetics is misaligned with reader interest.
  • Telehealth services that prescribe rapamycin without baseline labs. Rapamycin requires LDL, ALT/AST, blood glucose, lipid panel monitoring. A service that ships it after a 5-question intake is a service that does not know what it is prescribing.
  • "Peptide stacks" branded as anti-aging cures. No human RCT supports stacking 4-5 peptides together for longevity benefit. The stack is the clinic's margin play.
  • Subscription-locked services with no easy cancellation. Longevity protocols are long-term commitments. The contract should not be.
  • Influencer-affiliated longevity brands without published medical advisory boards. If the only credibility signal is a celebrity endorsement, the medical layer is too thin.
